Instructions for Completing the Undergraduate Registration Form

Make sure that you comply with the admission requirements regarding your field of study before applying for registration. Students must complete the forms and fax it to us for a first review.  

The following instructions will serve as a useful guide to explain and emphasize important points throughout the registration form. Questions on the registration form that are not listed in the guide below are marked as self-explanatory.  

Question 1a): Proposed Qualification (eg. BCom, LLM, Phd, DEd).

Question 1b): Specialization (eg Computer Science).

Question 2: Qualification Code (eg 02011).

If you are registering for a paper/module/course for non-degree purposes (NDP) only, please use the appropriate code below.

Question 3 - 5: Surname, Initials and title

Your surname must be given as it appears on the Certificate of Canadian Citizenship or on the SIN card required for admission.

Note The Following:

  • A woman who supplies her married surname here must submit a copy of her marriage certificate (or divorce papers where applicable) if her admission document was issued in another surname. See Question 3.
  • If your surname and/or first names have been officially changed, you must submit original evidence thereof. Your date of birth and your previous surname and first names, as well as your present surname and first names must all appear on this document. (Affidavits, birth certificates, passports and/or identity documents are NOT acceptable for this purpose.) See Question 3.
  • The University deems correct only the spelling and sequence of your surname and first names as they appear on your admission document.

For Title Only, the following may be used: Mr., Mrs., Miss, Ms., Dr., Prof., Rev., Pastor, Br., Fr., Sr.

Only the spelling and sequence of your first names as they appear on your admission document are accepted as correct.

Question 7: Maiden name and/or previous surname(s)

This must be answered only by the following:

•  A married and/or divorced woman; or

•  A student (male or female) whose surname has been officially changed.

If you have any serious physical disabilities which could affect your studies, please indicate your answer with the appropriate code in the block.

Note that all students with visual impairments must complete a questionnaire, which will be supplied to them on registration each year.

A physically handicapped student who is unable to write his/her examinations at an approved center must apply to the Academic Registrar in writing every year before March 1st for the May/June semester examination, and August 30th for the October/November or January/February examination, in order to receive permission to write at a special center. In this case, the student must submit a medical certificate specifying the nature of his/her handicap and are normally required to pay the commissioner's fees.

Question 14: Postal, Physical Address                 

It is compulsory for all students to supply all Postal and Physical Addresses. Academic correspondence and parcels with course materials are sent to the address given. Write the name of the country and the corresponding  postal code.

Question 15: Examination Center Code

The name of the center where you wish to write your examinations) and its code must be written in the appropriate blocks. For other centers not listed below, please contact us.

Give particulars of all previous registrations at Unisa and/or another University or institution for tertiary education (e.g. a Technikon, a Nursing College or teachers' college), starting with the most recent registration.

Question 31: If you have completed formal courses at another accredited university or university of technology, you can apply for exemption from study units already passed (conditions apply); you must, however, do this as part of the application process before you register. If yes, answer Question 32 as well.

•  Personal Cheque - If you choose not to use your credit card, you can mail us a cheque with your payment for application and tuition fees.

Choosing any option from above will get you a detailed statement directly from UNISA. Note that if you decide to cancel your studies at a later stage, you will be liable for 50% of the first semester/year fees.

Please fax or mail us your completed application form (one page only), along with your required documents (see below), for first review.

 Submission of your documents:

While we review your application and capture your information on UNISA system, you must mail us your original transcripts and all your complete academic record. Please also mail us a notarized ID document (Passport will do). Faxed or e-mailed documents will not be accepted as originals.  

Documents to be submitted:

•  If you did not obtain the qualification prescribed for admission at this University, you must submit an original transcript of your complete academic record for the relevant degree (issued by the REGISTRAR of the concerned university). In the case of a completed masters degree, the percentage/symbol obtained or the standard reached must be indicated on the transcript. Since the Head of the Department may demand your entire academic record, you are advised to submit a transcript of the academic record for your bachelor's degree, as well as your honor's bachelor's degree (if not obtained at UNISA).

•  A notarized document of your identity such us passport or birth certificates reflecting your full names and date of birth are also required for submission. The copy must be certified by a SA Embassy, Consulate, High Commission, Trade Mission or Public Notary.  

Matriculation exemption:

Candidates who first obtained a senior or school-leaving certificate (Grade 12) without exemption/university admission, and who then completed an approved three or four-year diploma may apply to the University for an Exemption Certificate (see: M30E form).

Mature-age conditional examination: Students of 23 years of age and over can apply for a certificate of conditional exemption under the special conditions applicable to non-matriculated persons.
